Participants in events held at Silver Tail Farm are requested to download this release form, sign it, and bring it with you to the event, or mail it ahead of time.

Once you’ve registered for an event, you are requested to download this form, print it, and mail it (along with your release) to us with your payment.
This form is NOT used for events that are produced by other organizations, where Silver Tail Farm is only hosting the event.
